Subject: school trip permission letter
Dear Dr. Clinton
We are looking for your permission for our daughter to go for a school trip to the dinosaur museum on 20 March 2012. As she recently had a fracture in her right arm, we are seeking your medical permission for that trip. She had the Firm plaster for 25 days. After that period the orthopaedic had confirmed her complete bone recovery, and only than her fracture was removed. Now she is absolutely fine and using her right hand quite well.
We are attaching all her reports along with the X-ray for your perusal. We are also attaching the list of the medicines currently used by my daughter. Please look into the matter and grant the permission to my daughter for her school trip.
