I was the bad news for me when I heard that your company had rejected my application for the medical insurance. I submitted my application in duly filled form. I was quite optimistic that I my application would easily be accepted and I would be granted medical insurance. But it is very sad that my application has been rejected by you.

However, I once again appeal you for the reconsideration of my application. It is quite possible that you might have misguided by someone. Therefore, it will be right to go through my application once again.

I am quite optimistic that the reconsideration of my application will move the decision in my favor and you will make a rational decision in regard of my application.
