Respected Mr. Creed,
We would like to thank you for your service at the sales department of our organization. As you are about to end your employment we wish to arrange for an exit interview with you.
The purpose of the exit interview is to take your views about your working experience, the management; factors that you feel will help to improve the working conditions and the shortcomings of the organization. We assure that the information provided you will be kept confidential and will not be mentioned in your records.
We have decided the exit interview date as 15th of April 2012, sharp at 11 in the morning. I am enclosing a questionnaire along with the letter that you need fill and bring on the day of exit interview.
